Pharmacy & Medical Equipment Insurance billing can be complex and requires cooperation between us, your doctors, you the patient and your insurance providers to meet the many requirements needed to support with the reimbursement of your home medical products and prescribed medications. Our pharmacy and medical equipment Billing team knows and understands the complicated insurance-billing requirements (including Medicare and Medicaid as well as most private insurances) and can help bill your claim and answer many your questions or direct to someone that can assist with your needs.
To help expedite the process prior to coming to our store location, please print or download, complete, sign and bring all required items listed below this is to help fulfill the proper billing requirements needed by your insurance company. For most insurance companies the following forms and/or documents are required:
• A valid prescription for the item(s): See Medicare Guidelines on “Proper Prescription”
• A copy of your Insurance Card(s) Front and Back
• A copy of your current Driver's License and/or other appropriate ID
• Order Intake Form (Click to download and print)
• Customer Information Checklist/medical release form (Click to download and print)
• Insurance verification will be handled by our knowledgeable staff
For Medicare only please include all the above, and the following:
• Most recent physician chart/progress notes, and/or
• Any notes or information related to your need(s)
